Required Insurance Information

We will need to know information about your plan, including:

• A copy of your valid insurance card
• Insurance billing address and phone number

Hospital and laboratory services will be paid to those providing organizations directly, not to our office.

Co-payments

Insurance plans may require a co-payment at the time of your visit.  Please be prepared with your insurance card and co-pay at the time of your visit.  We accept cash, checks, and most major credit cards.

Authorization

Some insurance plans require that you get authorization from them before you visit us.  if you have any questions, please contact our clinic and we help walk through the process with you.
